TREAT Steward™ is a comprehensive tool for implementing an Antimicrobial Stewardship Program. It links everything together, from intelligent recording of information and patient-specific treatment advice, to management reports and statistics. The main task of TREAT Steward™ is to help improve clinical outcome of patients with infections while preserving the effectiveness of antimicrobials for future generations. Keeping the consumption of antimicrobials and in particular, the broad-spectrum types, to a minimum is a key part of antimicrobial stewardship. TREAT Steward™ takes every piece of infection-relevant information into account in order to create consistency, assuring quality in the management of patients with infection and the use of antimicrobials. It gives an opportunity to improve treatment and follow-up on patients with infections. TREAT Steward™ is adapted to the antimicrobial policy of your hospital and helps infectious-disease specialists and management to control and monitor antimicrobial usage, cost and clinical performance.

TREAT Steward™ consists of a range of modules grouped according to three main areas of focus: quality assurance, decision support, and statistics and surveillance.

– Binds all infection-relevant information together for the specialist

The Worklist acts as a dashboard for the experts at the hospital, giving an overview of all patients receiving antimicrobial treatment, as well as those for whom antimicrobials should be considered. It provides an opportunity to review guideline compliance, usage of restricted antimicrobials and to monitor high-risk patients. The Worklist puts the hospital’s stewardship team in full control.

– Supports reviewing of the temporal course of an infectious episode

TREAT Steward’s Inspection Tool provides an elegant solution to display the temporal course of all infection-relevant information. This functionality is targeted at the specialist team, and allows them to review specific infectious episodes in greater detail. The episode history includes a complete overview of antimicrobial treatments, microbiology, vital parameters, biochemistry, hematology, blood gases and liver function indicators.

– Makes an excellent classification of the septic- and non-septic patient

TREAT Steward’s risk assessment adds an additional layer to the information contained in the Worklist. This functionality allows the specialist to identify which patients should be eligible for rapid diagnostic testing, and place extra focus on them to ensure expedited results. The risk assessment is based on a partial model of the systemic inflammatory response and is well-calibrated in predicting bacteremia and 30-day mortality. The figure below shows how the clinical evaluation by the A-team and risk assessment support the clinical work with diagnostic- and antimicrobial stewardship.

TREAT Steward™ aids the clinician in writing patient records. Full integration with hospital IT systems means that minimal data entry is required, and a structured, editable free text infection note is generated and sent back to the EHR. The burden of data entry is lightened while at the same time the quality of documentation is improved. TREAT Steward™ helps to ensure that all infectious patients are examined and treated according to the same procedures and at the same quality at all times.

Supports the communication of local antimicrobial guidelines and policies

TREAT Steward™ implements antimicrobial guidelines in an intelligent, rational and intuitive way. Through a calibration process, the decision support algorithm is fitted to local guidelines and preferences; in this way, the use of TREAT Steward™ helps to reinforce the guidelines and educate clinicians on best practice. Based on the examination of the patient and the information available in their EHR, TREAT Steward™ suggests likely diagnoses and evaluates the severity of illness as well as presenting the likely causative pathogens. On this basis, a list of recommended treatments is generated. All of this information is displayed for the clinician on the Summary and Treatment page.

- Making hospital guidelines available at point of care

In combination with the TREAT Steward™ decision module the user has easy access to the most recent evidence-based knowledge about infections, microbiology and antimicrobial treatments. This feature makes is easy to present hospital guidelines to clinicians in an intuitive way when the treatment decisions is made. Treatment guidelines are automatically displayed for the most likely diagnosis and usage guidelines are displayed for the top recommended antimicrobial treatment. The user can then search and navigate through additional pages for other diagnoses and/or treatments.

– Finding correct dosage, frequency and duration is easy with TREAT Steward™

In addition to advice on diagnosis, pathogen distribution and treatment, the system modifies the daily antimicrobial dose according to the patient’s kidney- and liver function, weight and the focus of infection. The user-friendly interactive interface of TREAT Steward™ helps the clinician to identify the optimal treatment for each patient with respect to dose, frequency, timing and duration of treatment.

The dosage module adjusts the standard antimicrobial ordination in relation to the patient’s kidney and liver function, weight, and the suspected or confirmed site of infection.

– Personalizes the institutional antibiogram for each patient

Using the institutional antibiogram as a starting point, TREAT Steward™ adjusts for whether a patient’s infection is community- or hospital-acquired, microbiological test results and whether they have recently received antimicrobial treatment. This innovative approach also allows the estimation of the probability of susceptibility to antimicrobials not included in standard testing.

Patient-specific antibiograms serve as a better starting point for the choice of empirical antimicrobial therapy than the institutional antibiogram.

– An educational review tool for tracking individual performance

The nature of empirical antimicrobial treatment means that a patient will often be moved to another department or even discharged before microbiological results are available. At this stage, any opportunity for follow-up by the same clinician is lost. TREAT Steward™ provides a functionality called My Patients, allowing the individual clinician to follow up on the clinical performance of their choice of empirical therapy and whether or not it covered the identified microorganism(s).

TREAT Steward™ generates measures and statistics of overall clinical performance, antimicrobial consumption and guideline compliance, supplying department managers and hospital administrators with valuable input to their decision making.
